Financial Information
Our goal is to provide excellent chiropractic care to you and your family. We want you to be comfortable dealing with financial matters, and believe open communication will enhance the positive outcome we all desire.
Our office is a fee-for-service practice, meaning that payment for fees incurred is due at the time services are rendered. For your convenience, we accept, cash, debit, personal checks, and all major credit cards.
If you have insurance we do want you to receive the maximum benefits. At the end of your first appointment, we will provide you with an invoice, called a superbill, which you can then mail to your insurance provider with one of your insurance forms. The address is typically on the back of your insurance card or you can call their customer service number for more detailed instruction. Dr. Casab is not a member of any HMO, PPO, Medicare, or any other provider networks. Therefore, any coverage you may have for services provided will be deemed “out of network coverage” by your insurance company. After their deductible is met, most patients can expect 50% – 80% of their visit to be covered, which means they are paying very little out-of-pocket for exceptional care.
